Liability Waiver

Please read the following wellness liability waiver carefully. Metisine Health provides wellness technology for general wellness purposes only — not medical treatment. Signing below records your acknowledgement.

  1. Metisine Health provides wellness technology — including hyperbaric, red-light, and molecular-hydrogen equipment — and related wellness sessions intended for general wellness, relaxation, and educational purposes only.
  2. These products and sessions are NOT medical devices or medical treatment. They are not intended to diagnose, treat, cure, or prevent any disease or medical condition. These statements have not been evaluated by the Food and Drug Administration.
  3. I understand the inherent risks of these wellness activities, which include but are not limited to: ear, sinus, or pressure-related discomfort and temporary vision changes from hyperbaric sessions; eye strain or skin sensitivity from red-light sessions; and that molecular hydrogen is a flammable gas that must be used only as directed. I accept that some risks cannot be eliminated regardless of the care taken.
  4. I confirm that I have reviewed the wellness contraindication checklist and that, to the best of my knowledge, none of the listed conditions apply to me, or that I have discussed my participation with a qualified healthcare professional.
  5. I understand that I should consult my physician before beginning any new wellness practice — especially if I am pregnant, have an implanted device, a respiratory or seizure condition, recent surgery, or any acute illness — and that I will stop a session and seek help if I feel unwell.
  6. I voluntarily assume all risks associated with using Metisine Health equipment and participating in wellness sessions, and I release and hold harmless Metisine Health, its owners, employees, and affiliates from any claims or liability arising from my participation, to the fullest extent permitted by law.
  7. I agree to indemnify Metisine Health for any loss or claim resulting from my failure to follow safety instructions or from inaccurate information I have provided. This waiver is binding on me, my heirs, and my legal representatives.
  8. I confirm that I am at least 18 years old (or am signing as the parent or legal guardian of the participant), that the information I have provided is accurate, and that I am signing this waiver of my own free will.
